CREC Athletics is moving its preseason student-athlete registration away
from paper packets to an online platform called Final Forms. We have
linked the parent playbook for Final Forms registration in English and Spanish
as well a video tutorial on how to complete the parent registration. All of this has also
been posted onto the CREC Athletics page.
Once a parent has created an account, they can begin completing the
forms. The student will also need to create an account after the parent has
created theirs.
Winter sports are scheduled to begin on January 19, 2020 and
all athletes will need completed registration in order to participate.
